PATIENT ADMISSION
Admission to the Hospital
Upon arrival at the hospital, a patient in a state of sudden deterioration in health is directed to the Hospital Admission Room, where a doctor conducts an examination and initial diagnostics. Based on the results, a decision is made whether to admit the patient to the hospital or determine further medical procedures.
Required documents:
- Referral (not required in case of emergency)
- Child’s health book
- Child’s PESEL number
- Complete medical documentation of the child’s previous treatment and current test results
- Health insurance registration information
- Patients from the European Union: ID card or passport, valid European Health Insurance Card (EHIC) or equivalent insurance certificate. In non-emergency situations, EU and EEA citizens may be referred to a hospital outside their home country for planned treatment after obtaining the appropriate referral. In such cases, admission is based on the S2/E112 form. Failure to present one of the above documents may result in the patient being charged for the procedure according to the official price list.
- Non-EU patients may be required—depending on the conditions of their Travel Medical Insurance—to cover the costs of treatment, which may later be reimbursed by the insurance company. The hospital will contact the insurer immediately after the patient’s admission to verify the method of payment.
